['klɪərəns, englisch; eigentlich »Reinigung«] die, -, Medizin und Physiologie: Reinigung einer bestimmten Blutplasmamenge von in ihr befindlichen körpereigenen oder künstlich eingebrachten Substanzen durch ein Ausscheidungsorgan (z. B. Nieren oder Leber). - Als renale Clearance (medizinisch kurz Clearance genannt) bezeichnet man die Blutplasma- oder Serummenge, die beim Durchfluss durch die Nieren je Minute von Harnstoff oder einer anderen harnfähigen (durch die Glomeruli der Nierenkörperchen filtrierbaren) Substanz vollständig befreit wird. Der bei der Nierenfunktionsprüfung ermittelte Clearancewert (Klärwert) ist das Maß für die Ausscheidungsfähigkeit und -geschwindigkeit der Nieren. Seine Größe hängt u. a. davon ab, ob die verwendete Testsubstanz in den Glomeruli der Nieren nur filtriert oder in den Nierentubuli auch rückresorbiert oder sezerniert wird. Die Clearance ausschließlich filtrierter Substanzen (z. B. Inulin) ist identisch mit der glomerulären Filtrationsrate (GFR), die Clearance zusätzlich noch sezernierter Stoffe (z. B. p-Aminohippursäure, PAH) identisch mit dem effektiven renalen Plasmadurchfluss. (Niere, Harn)
